MDS Coordinator / Nursing Supervisor

Hillcrest Health & Rehabilitation Center is currently seeking an?MDS Coordinator/Nursing Supervisor.


The MDS Coordinator/Nursing Supervisor will ensure that the highest degree of quality care is maintained at all times, including assisting in rendering?care to address resident needs with recognized nursing techniques, procedures, and established standards based on the scope of nursing.


Additionally, the MDS Coordinator/Nursing Supervisor is responsible for information gathering and completing computer generated assessments, care plans and supporting documentation on residents. ?
The successful candidate will posses?good computer skills, be detail oriented, a quick learner and able to work independently and efficiently to meet deadlines.??

  • Communicate effectively and appropriately with residents, families, and members of the health care team.


  • Recognize and respond to the needs of the residents.


  • Communicate a sense of caring, concern, and dignity for residents.


  • Maintain a positive, upbeat, and encouraging attitude toward residents.


  • Effectively manage time to perform the essential functions of the job.


  • Function in the healthcare team as a resident advocate.


  • Ability to function in dynamic environment under time constraints and task deadlines


  • Ability to work with and motivate others.


  • Make prompt and accurate judgments with regard to MDS Coordinator duties


  • Observe and assess at the MDS Coordinator level the physical, psycho-social, and safety needs of residents.


  • Coordinate and prepare MDS Assessments, Care Plans, and supporting documentation for residents.


  • Prepare and update assessments, Care Area Assessments, and Care Plans as necessary.


  • Participate in Care Plan meetings with residents and family members.


  • Be aware of resident?s exit to and re-entry from hospitals.


  • Communicate with co-workers at all levels to adequately meet the needs of residents.


  • Participate in facility committees and audits, as needed.



Required license or certification:




  • Valid RN license in the state of Kentucky. ?


  • Completion of relevant state Board of Nursing approved.


  • Experience in Long-Term Care is preferred.?

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
